Raffle Proposal Request

A university department, entity or organization planning to conduct a raffle must complete this form and receive approval prior to the raffle being conducted. This form must be completed per the Raffle Policy For ÃÛÌÒ´«Ã½ Fundraising Efforts.

Instructions

  1. Complete all required fields and click Submit Request.
  2. You will receive a confirmation email at the email address you provided on the form.
  3. Financial Management will upload your request to Adobe Sign to obtain all necessary signatures.
